School transition mat

I have created this file to help with transitions to school and next academic year groups. I have tried to keep this mat easy to read so information doesnt get too overwhelming, by colour coding key information. The aim was to ensure important information is given to the pupil and is easier to understand. 
The centre space ot to fill in "My transition to" here you put nursery, reception, year 1 etc. The rest of the squares around the centre have spaces to fill in key information.
